OBR Employee Change Form
The link below leads to the OBR Employee Change Form. This form is used to update Human Resources about changes in an employee’s personal information, compensation, or employment status.
This form can be downloaded and filled out, or printed and either scanned or mailed in. Email forms to hr@obrcoolingtowers.com or mail forms to our Northwood, Ohio office. HR will reach out once the form is received.
To enroll for online banking with State Bank, you will need your HSA Account Number. Contact hr@obrcoolingtowers.com to get your HSA Account Number. You will also need to enter a PIN while enrolling. This PIN is the last 4 digits of your Social Security Number.
Note: You opt in to having an HSA during annual Open Enrollment. If you did not opt in to an HSA, you don't have one through OBR.
