OBR Employee Change Form

The link below leads to the OBR Employee Change Form. This form is used to update Human Resources about changes in an employee’s personal information, leave of absence, compensation, or employment status.

This form can be downloaded and filled out, or printed and either scanned or mailed in. Email forms to hr@obrcoolingtowers.com or mail forms to our Northwood, Ohio office. HR will reach out once the form is received.
